A survey has been emailed to all families from the Berlin Central School District regarding the 2021-2022 school year.

Attached to the survey is a form for a medically fragile exemption to the in-person learning.
Please print and fill out this form, if applicable, and send the completed form directly to the contact person indicated on the form.
