Important updated sports information for all students that will be entering 7-12th grades for the 2021-2022 school  year. For any athlete choosing to participate in a sport for the 2021-2022 school year, the health office must have an up to date physical on file within 1 year of the start date of the sport season in order to begin practice. For fall JV/Varsity sports, this means that your child’s physical must be dated on or after August 23, 2020 to be considered up to date for this sports season. For modified fall sports, this means that your child’s physical must be dated on or after September 7, 2020 to be considered up to date for this sports season. Health appraisal forms for your doctor to complete have been mailed home. You may also find forms below or on your district’s website. You may have your child’s health examination form faxed to the your child’s health office or you may mail it to the school. We want to make sure all athletes are eligible to practice on the first day.

Additionally, both schools are pleased to be able to offer physicals for those students who are unable to get appointments with their primary doctor.  For Berlin students, physicals will be offered at the BMHS by appointment only on August 10 between 9:30-1:00.  If you would like to take advantage of this option, simply contact Michele Corsey at mcorsey@berlincentral.org to schedule an appointment.  Appointments will be made on a first come, first serve basis. If your child will be receiving a physical by their primary physician, please notify your child’s school nurse of the  appointment date.
